Reason for complaint :
Incident #1 - On January 6, 2001, at approximately 10:00 p.m.,Mary Smith left a confused, physically disabled patient, Mr. C.,hospital # 0496832, unattended in the bathroom for 45minutes. Despite repeated calls for help, which were brought tothe attention of Mary Smith by two other patients, Mr. S. and Ms.M., Mary Smith refused to leave the desk where, according to Mr.S., she was writing on charts. Mr. C. finally attempted to leavethe bathroom without assistance. This attempt led to a fall whichresulted in a fractured right hip. Appendix A and B are anecdotalnotes from my discussion with Mary Smith on July 7, 2001.
Incident #2 On July 23, 2001, between 9:00 and 10:00 p.m. inRoom 2463 west, Mary Smith was seen slapping a patient, Ms.A., Hospital # 038471, on the right leg when Ms. A. refused towalk from her chair to her bed. Jane Doe, LPN, was caring for apatient in the next bed to Ms. A. Appendix C is a handwrittennote from Jane Doe describing the incident.
Following a July 7, 2001 and a July 24, 2001 letter of warning(Appendix D), Mary Smith's employment with AA Hospital wasterminated on August 5, 2001. (Appendix E)
