Whereas, disputes and differences have arisen between theparties with respect to that certain contract entered intoby said parties and executed on [date] , a copy ofwhich is attached hereto as Exhibit A, both parties haveagreed to settle said disputes and differences by executingthis mutual release.

This is a typical letter. You have to add your company's name with full address. And the receiver's name and full address also should be added in the place allotted for that purpose.
