Personal letters reveal close and informal relation between the writer and the reader. In most of the books on letter writing, I don’t find use of any salutation to the reader. The write begins the letter simply with….

The postman has just delivered your letter ……………..
I have received your letter just now.
We received your letter yesterday.
But there is no mention of Mr. / Mrs. / Ms.
Please note, a letter should be written in such an informal way that the reader feels as if you are sitting before him.
If a son is wrting a letter to his mother whom he calls BAA then he should greet her as following.
My Dear Baa,
My Dearest Baa,
My Loving Baa,
And if you are used to covey JAI SHREE KRISHAN to your mother whenevy you meet her or there follows some other tradional way of saluting the elders in your house, then you most use….

If you are writing a letter to your grand mother, whom you call DADI MAA then you should greet her as following…
My Dearest Dadi Maa

This is a typical letter. You have to add your company's name with full address. And the receiver's name and full address also should be added in the place allotted for that purpose. Without those primary details no letter carries any significance. So, you are requested to provide all those important details. Failing which the letter will make a wrong impression about your company and you among the receivers of your letters. This is the most unwanted attitude on your side in dealing with the clients who are the most valued assets of any business organisation. If necessary, leave those spaces blank.
